Identity Verification
Students may be required to verify their identity. If chosen to complete this process, financial aid will not be determined at Empire State University until this requirement has been satisfied. Students selected to complete this identity verification have two options to do so and must choose and complete one or the other.
Option 1: Book a Virtual Meeting with a Financial Aid representative. During this meeting you must have your camera on and be visible. You will be expected to present your unexpired government issued identification during the meeting. Meetings can only be booked for verification of identity purposes.
Option 2: Download and complete the appropriate year form below. Students will be required to meet with a notary in person to present their identification. Submit the original completed and notarized form along with a copy of the unexpired government-issued ID to our office. Please note copies, emails or faxes will not be accepted.
